call my office (858 560 5609). After scheduling your initial appointment, please return to this website (www.danielgardner.yourmd.com) to take the following steps: 1. Create an account. (Click on the link "Create an Account" under "Patient Login" on the upper left side of the home page.) 2. After your account is created, you may choose to complete a Personal Health Record, which is a secure and confidential personal record of your medical history. 3. Download, complete, and return the forms in the file, "New Patient Forms.pdf" (See below). 4. You may return the completed forms to me by mail or fax.
